Onondaga County, New York, plans to spend $185.9 million on employee and retiree benefits in 2027, a $24.7 million increase over its 2026 adopted budget of $161.2 million, according to county budget documents. The 15.3% jump makes benefits one of the fastest-growing line items in a proposed $1.7 billion spending plan across all county funds.

Healthcare drives most of the increase. The county projects health benefits alone will cost $91.7 million in 2027, up from $75.1 million this year — a $16.6 million, or 22%, rise. Budget documents attribute most of that growth to a higher rate for the county's Medicare Advantage plan, alongside significantly higher prescription drug costs and a higher state retirement contribution rate.

For HR and benefits leaders, the numbers illustrate the compounding pressure retiree health programs place on public-sector budgets. Onondaga County self-insures its health, prescription, dental, unemployment and workers' compensation programs, meaning rate increases flow directly into its budget rather than being buffered by a carrier's premium structure.

The county is not covering the full increase from current-year revenue alone. The proposed budget appropriates $2 million from its fund balance — money already held in reserve — to offset employee and county contributions toward benefits. That approach funds part of the program from reserves rather than relying entirely on ongoing revenues, a decision that shifts cost pressure into future budget cycles.

The benefits line sits within a broader context: county officials are also planning what would be their ninth consecutive property tax rate cut, with sales tax revenue up 36% adding $133 million.

The Ways and Means Committee will hold a public hearing on the proposed 2027 budget, capital program, budget message and water rates at 5 p.m. Thursday, Oct. 1, in the Legislative Chambers at the Onondaga County Courthouse in Syracuse. Written public comment will remain open until 11:59 p.m. that day, with submissions directed to [email protected] under the subject line "10.1.26 Public Hearing."

The county Legislature is expected to continue reviewing the proposed budget before final adoption later this fall. If adopted as proposed, the 2027 plan would lock in benefit cost assumptions — including the higher Medicare Advantage rate — that will shape the county's total rewards decisions for the coming year.
